Adding new fields to Progress Note csv for Power BI export

Add be "ProgressNoteStatus" and "ProgressNoteLockDate" fields to the PowerBI Extract for the Progress Notes table.

    The 'Date - Locked' field has been added to the Power BI 'Progress Note' table. The status of a Progress Note (e.g., Draft, Completed, Locked) can be determined via the 'Form Status ' table.

    I would recommend that this entry be considered as implemented.
